Abstract: ملخص الدراسة هدفت الدراسة التعرف إلىمعوقات تطبيق شهادة اعتماد اللجنة الدولية المشتركة لجودة الخدمات الصحية (JCI) في مستسشفى الهلال الأحمر الفلسطيني (فرع الخليل)، حيث تناولت الدراسة ثلاثة محاور، ناقشت الباحثة من خلالها المعوقاتمن أجل الحصول هذه الشهادة، وكانت محاور الدراسة الرئيسية هي:(التزام اﻹدارةاﻟﻌﻠﯾﺎﺑﺗطﺑﯾقﻧظﺎم ال (JCI)والثقافة التنظيمية في المستشفىو العنصر البشري)،وتم اعتمادأسلوب المسح الشامل، حيث أخذت الباحثة مجتمع الدراسة كاملا كعينة للدراسة وتمثل المجتمع بمستشفى الهلال الأحمر الفلسطيني (فرع الخليل). وانطلاقا من طبيعة الدراسة فقد استخدمت الباحثة المنهج الوصفي الإستكشافي، الذي يعتمد على دراسة الظاهرة -موضوع الدراسة - في الوقت الحاضر كما هي في الواقع، وتحليل النتائج، ومن أجل تحقيق هذا الهدف اعتمدت الباحثة على أداتين لجمع بيانات الدراسة، وهي: الاستبانة، والمقابلات . وقدتم توزيع الاستبانة على موظفي المستشفى وأعضاءالهيئة الإداريةجميعهم، والبالغ عددهم(211)، وقد تم استرداد 180 إستبانة، تم تحليل 160 إستبانة صالحة للتحليل، أي بنسبة استرداد قدرها (%85)، وقم تم إدخال هذه البيانات إلى برنامج(SPSS) ومعالجتها للخروج بالنتائج. وأجرت الباحثة مقابلات مع أعضاء الهيئة الإدارية ورؤساء الأقسام ومسؤوليها في المستشفى-محل الدراسة -وقامت بتحليلها يدويا، وقد توصلت الدراسة إلى مجموعة من النتائج،أهمها: • وجود معوقات مرتبطة بالتزام الإدارة العليا بتطبيق الJCI)) متعلقة بتدني درجة وعي الإدارة العليا بشهادة اعتماد الJCI))، وأهميتها على تحسين جودة الخدمة الصحية المقدمة وتدني استعداد الإدارة العليا لدعم هذا الاعتماد ماليا، وتدني استعداد الإدارة العليا أيضا لإعطاء الوقت اللازم لتطبيق هذه الاعتماد. • وجود معوقات لتطبيق ال JCI))مرتبطة بالثقافة التنظيمية في المستشفى، فيما يتعلق بعملية إدارة الاتصالات والمعلومات، من أهمها:القصور في الاتصال بين المستشفى والمرضى وعائلاتهم وبين المستشفى والمجتمع. • وجود معوقات لتطبيق ال JCI))مرتبطة بالعنصر البشري المتمثل بالتدريب والتوظيف، من أهمها:عدم توفير الإدارة العليا لبرامج تدريب وتطوير للعاملين، وعدم استعداد المستشفى لجذب كوادر متخصصة بتطبيق ال (JCI) وتوظيفهم. وقد قدمت الدراسة عدة توصيات أهمها إنشاء قسم خاص يعنى بإدارة الجودة الشاملة في المستشفى،ليكون اتصالها بالإدارة العليا كحلقة وصل مع المستويات الإدارية الأخرىوتفعيل دور الإدارة العليا في المستشفى، وزيادة حجم مشاركتها في تبني تطبيق أنشطة تحسين الجودة الشاملة والعمل على تدريب مقدمي الخدمة في المستشفى وتأهليهم وتمكينهم وتوعيتهم بمعايير ال (JCI)، وتحفيزهم عن طريق ربط نظام الحوافز والمكافات بإدارة الجودة الشاملة،تبني خطة عمل استراتيجية شاملة تهدف إلى نشر ثقافة الجودة الشاملة في المستشفى وفلسفتها ومفاهيمها. وقد قدمت الباحثة آلية لتنفيذ للحصول على شهادة ال (JCI) من خلال عقد ندوة في المستشفى، بحضور أعضاء الهيئة الإدارية ورؤساء الأقسام، لمناقشة معوقات التطبيق وسبل حلها وتجاوزها. Abstract The study aimed at identifying the obstacles for applying the JCI accreditation certificate in the Palestinian Red Crescent Hospital – Hebron Branch. The study investigated three aspects and discussed the obstacles to obtain this certificate. The main aspects are (Senior management’s commitment to apply the JCI system, organizational culture in the hospital, and the human element). The comprehensive survey method was adopted where the researcher took the entire study population as the study sample that is represented in the Palestinian Red Crescent Hospital – Hebron Branch. Based on the nature of the study, the researcher used descriptive exploratory methodology, which depends on studying the phenomenon under investigation as it is in fact and analyse of the results. In order to achieve this objective, and to collect the study data, the researcher relied on two methods that are the questionnaire and interviews. The questionnaire was distributed to all hospital staff and members of the administrative board and whose number was (211). A total of (180) questionnaires were receivedback, and (160) questionnaires were analysed, and with a recovery rate that reached 85%. After that, the data was processed and analysed using SPSS program. The researcher conducted interviews with the members of the administrative board, the officers and department heads in the hospital under investigation, and then analysed them manually. The study concluded with a number of outcomes, most importantly: • The existence of obstacles concerning the commitment of senior management to implement the JCI that may be related to the low level of awareness of senior management with the JCI accreditation certificate, and its importance in improving the quality of health service provided. Low willingness of senior management to support this accreditation financially, and low readiness of senior management to give the time necessary to apply this accreditation. • The existence of obstacles to the implementation of the JCI is linked to the organizational culture of the hospital in relation to the communication and information management process, the most important of which is the lack of communication between the hospital, the patients and their families, and between the hospital and the community. • The existence of obstacles to the implementation of JCI linked to the human element that is represented in training and employment, the most important of which is the lack of senior management’s provision of training and development programs for staff, and the unwillingness of the hospital to attract and employ cadres specialized in the implementation of the JCI. The study presented several recommendations; it indicated to the necessity to establish a special department on Total Quality Management(TQM) in the hospital, which will be in contact with senior management as a link with the other administrative levels. Additionally, it highlighted the importance to activate the role of senior management in the hospital and increase the size of its participation in adopting the application of the activities, which will improve the overall quality, training, and empower service providers at the hospital and raise their awareness of JCI standards. It will also work to motivate them by linking the incentives and rewards system to the overall quality management, and the adoption of a comprehensive strategic plan of action aimed at spreading the culture, philosophy and concepts of overall quality in the hospital. The researcher presented a mechanism to obtain the JCI certificate in a workshop in the hospital in the presence of members of the administrative body and heads of departments to discuss the obstacles of implementation and ways to solve and overcome them.
